Lost all or most teeth on one arch? The All-on-4 protocol allows a permanent fixed prosthesis to be placed on just four implants in a single surgical day. You leave the clinic with a full set of teeth.
Book a consultationAll-on-4 is a modern full-arch rehabilitation protocol for edentulous jaws in which a complete fixed bridge is supported by four implants. The two anterior implants are placed vertically, while the two posterior implants are tilted at 30–45 degrees. This approach bypasses areas of reduced bone density and eliminates the need for bone grafting in the majority of cases.
iDent performs All-on-4 as a true same-day procedure: you come in for surgery in the morning and leave with a temporary fixed prosthesis the same evening. The definitive prosthetic restoration is fabricated after osseointegration — typically 3–4 months later. Surgical planning is performed on the PaX-i3D Smart 3D CT scanner, which provides a precise picture of bone volume and nerve position before the first incision.
We perform a 3D CT scan on the PaX-i3D Smart: bone volume and density are assessed, together with the proximity of the maxillary sinus and inferior alveolar nerve. Based on the CT data the surgeon creates a digital surgical plan and determines the precise coordinates and angulation of each implant.
Surgery is performed under local anaesthesia or sedation. Any remaining teeth are extracted, and 4 implants are placed according to the pre-planned template. Total operating time is 2–3 hours. Impressions are taken immediately after surgery and sent to the dental laboratory.
A few hours after surgery, the clinician fits a temporary acrylic prosthesis. You leave the clinic with a fully functional set of teeth. The temporary restoration protects the implants and helps you adapt to your new bite.
After complete osseointegration at 3–4 months, the definitive prosthesis is fabricated from zirconia or metal-ceramic. A final occlusal adjustment is performed. Written warranties are provided on both the surgical and prosthetic phases.
All-on-4 is not the only full-arch rehabilitation protocol — for some patients All-on-6 is a better fit. At your consultation the surgeon will review the CT scan and recommend the optimal solution for your specific clinical case. First consultation is free.
Most patients treated with All-on-4 report a dramatically improved quality of life: they can eat solid food again, smile with confidence and stop thinking of their prosthesis as something temporary. With proper hygiene and regular check-ups, implants can serve for decades.
Surgery is performed under local anaesthesia or sedation — the patient feels nothing during the procedure. Mild swelling and discomfort for 3–5 days afterwards is normal and well controlled with the pain relief prescribed by your surgeon.
The surgical day and temporary prosthesis take just one visit. The final permanent prosthesis is placed 3–4 months after osseointegration. The full cycle from first consultation to permanent result is 4–5 months.
In most cases no. By tilting the posterior implants at 30–45 degrees, we can utilise areas of adequate bone volume even where significant atrophy is present. The definitive answer comes after analysis of the PaX-i3D Smart 3D CT scan.
