Due to insufficient space in the jaw it frequently erupts incorrectly — at an angle toward adjacent teeth, partially covered by gum tissue, or not breaking through the surface at all (impacted tooth). Such a tooth becomes a chronic source of inflammation, pressure and pain.
At iDent, wisdom tooth extractions are performed by a surgeon experienced in complex atypical procedures. A mandatory preparatory step is a 3D CT scan on the PaX-i3D Smart. The CT shows the precise angulation of the tooth, root morphology and the distance to the inferior alveolar nerve. This information allows the surgical approach to be planned in a way that minimises trauma and the risk of complications.
The surgeon reviews the clinical picture and orders a 3D scan on the PaX-i3D Smart. The CT displays the precise three-dimensional position of the tooth, bone status and proximity to critical anatomical structures. The surgeon then explains the surgical plan and answers all questions.
A nerve block is administered, anaesthetising the entire region rather than a single tooth. The injection is virtually painless and takes effect in 3–5 minutes, remaining effective for the entire procedure. Sedation is available on request.
Depending on complexity, the surgeon may incise the gum and section the tooth or extract it in one piece. The socket is irrigated with an antiseptic and filled with osteoplastic material where indicated to promote better healing. The wound is closed with resorbable sutures.
The surgeon gives detailed post-operative instructions: when to eat, how to rinse, which symptoms are normal and which require early review. Follow-up appointment at 7–10 days or sooner if needed.
After wisdom tooth removal, swelling and mild pain in the first 1–3 days is normal. A cold compress, pain relief as prescribed and antibiotics (if indicated) are the standard post-operative protocol following a complex extraction. Most patients return to their usual routine within 2–3 days.
If a wisdom tooth is asymptomatic and correctly positioned, removal is not necessary. The decision is based exclusively on clinical data and CT findings — not as a precaution.
The procedure is performed under a reliable nerve block — the patient feels pressure and vibration but no pain. After the anaesthesia wears off (2–4 hours), mild aching is possible and responds well to standard over-the-counter pain relief. Most patients describe the sensation as tolerable.
The PaX-i3D Smart CT shows the exact three-dimensional position of the tooth, the number and shape of its roots, and the distance to the inferior alveolar nerve and maxillary sinus. Without this information the surgeon cannot accurately plan the incision and approach. CT reduces the risk of complications and shortens operative time.
Two to three hours after the anaesthesia wears off and normal sensation is restored. For the first day or two, food should be soft and at room temperature: porridge, mashed potato, yoghurt. Hot, hard and spicy foods should be avoided for 3–5 days. Your surgeon will give specific recommendations immediately after the procedure.
