Unlike gingivitis, which involves only the gum tissue, periodontitis destroys the alveolar bone and periodontal ligaments. Left untreated, this is an irreversible process: the tooth gradually loses its anchorage and eventually falls out — even in the complete absence of decay.
The good news: at mild and moderate stages, periodontitis can be arrested with conservative treatment — no incisions, no surgery. At iDent we use a three-phase protocol in which each step has a specific goal and outcomes are verified by re-probing the pockets after the active phase.

Supragingival scaling (Phase 1)
An ultrasonic scaler removes mineralised plaque and calculus above the gumline and in the gingival margin area. This eliminates the primary source of inflammation. After the first session, bleeding typically reduces within a few days. Price: UAH 1,820.

Subgingival scaling and root planing (Phase 2)
The most critical phase: using hand curettes and fine ultrasonic tips, the clinician debrides root surfaces deep within the pockets. The procedure is performed under local anaesthesia. Removing subgingival calculus eliminates the bacterial reservoir and allows the gum tissue to reattach to the tooth. Price: from UAH 1,820.

Polishing and deep fluoride therapy (Phase 3)
The final phase — root surface polishing to inhibit plaque reattachment and deep fluoride application to strengthen root cementum. The gums begin to tighten around the teeth as pockets become shallower. Price: UAH 2,140.

Review appointment at 6–8 weeks
The clinician re-probes pocket depths and compares the readings against the baseline periodontal chart. Sites that did not respond to scaling are treated with vector therapy or other adjunctive measures. Supportive therapy is scheduled every 3–6 months based on clinical response.

The active phase consists of 2–3 sessions over 2–4 weeks, plus a review appointment at 6–8 weeks. After that, supportive therapy is scheduled every 3–6 months. Your clinician will specify the exact number of visits after compiling your periodontal chart.
At mild and moderate stages — yes. The conservative protocol (three-phase scaling plus vector therapy where needed) arrests inflammation and reduces pocket depths without surgery in the vast majority of cases. Surgery is considered only for pockets exceeding 6 mm that have not responded to non-surgical treatment.
